What is a Success Plan?
Success Plan: What is it and how is it used?
The Success Plan is a tool that is used by your instructor to assess, problem-solve,
plan and take appropriate actions when you are having any issues that affect
your academic performance.
You can be proactive, and can help yourself get on the right track by using this form. You can connect early with the activities and support that will help you achieve good academic standing.
Students who are on “academic probation” may be required
to use this plan.
Self-assessment and self-referral to services:
Sometimes, the process of focusing on these issues on your own will be
enough for you to self-assess and come up with the solutions to your own
issues. You can always go directly to the services that you believe would
be helpful to you.
Meeting with your program coordinator or advisor:
You may prefer to talk to someone to clarify what your issues
are and how to get help with them. You can use this form to consider the
reasons for your academic difficulty. You may also want to go as far as
developing your own solutions and strategies for success. Print the form
and make an appointment with me or your advisor to go through it with
you. We can help you work out a final action plan and get set up with
appropriate services.
